4CM_phpContact is a very simple script for having a "contact us" page at your site. It allows you to add as many fields as youd like. It will send you the results of the submission, as well as sending a confirmation email to the user who submitted the data. It is very easy to install. It includes a small instructions for setting up, and all the needed files.

phpFormGenerator

phpFormGenerator is an easy-to-use tool to create reliable and efficient web forms. No programming of any sort is required. It has the ability to create up to 100 form fields, and to add a variety of field types including text boxes, drop down selection menus, check boxes, radio buttons, passwords, freeform text areas, and file upload!. All field types are highly customizable. The form data can be sent to a provided email address, or stored in a database table (currently MySQL). A file-based database system is also provided for those who dont have access to a MySQL database server. Also included is an administration portal to be used with the stored form data. This management tool works with both the MySQL database and the file-based database created by the program.

PHPFormmail is a PHP script, written in French, which allows you to send the contents of a form by email. It also features: file attachment support, ability to make copies of recipients, configurable redirecttion page and option to define the forms priority (Urgent, High, etc).

Just add your email address (or multiple addresses) and youre done. This will work with any existing forms you have without any modifications to your html. Your visitors will get a thank you email sent to them soon after submitting your form. The autoresponder even sends a smart reply with the first name of your visitor in the message. You can even send visitors a receipt. Email addresses and phone numbers are checked for validity. Error pages can be easily customized to look like the rest of your website. You can turn on an option that sends you the browser type and IP of your visitor. The script will even handle MULTIPLE file attachments and screen out potentially dangerous files. Attachments can be saved on your server or just forwarded to you by email. Anti-spam security features are hardwired into the code to protect your site from abuse.
