47. Dynamic Survey System

Survey system application has been developed as a senior project in the CIS program at UNF under Dr. Solano. It was developed for the Advising Department but to this day has not been implemented on their web site due to lack of funds. With Dr. Solano¡¯s and my coauthors?permission I am making this program available under the GPL license. Survey is the more complex of the two. The survey system allows the administrator to build multiple surveys dynamically (although in a sometimes restricted manner) from survey description, to different kind of questions and different choices to even specifying which questions are required to be answered. The administrator may then create extremely detailed reports from the filled out questionnaires and even export them to Excel. Visit my site to view a live example of the client and admin sides.
